Requirements to obtain a US passport:

• Complete passport application Form DS-11 or Form DS-82 (renewing)
• Show proof of citizenship.
• Provide proof of identity.
• Provide two (2) passport photos.
• Make proper payment.
• Take all documents and payment to an application acceptance office.
• Show the acceptance agent your social security number.

You can obtain a US passport if any of the following apply:

• You have never had a passport (if already have a passport you can renew it)
• Your last US passport expired more than 12 years ago
• Your most recent US passport was lost or stolen
• Your most recent US passport was severely damaged

You can renew your passport by mail if...

• your present passport is not damaged
• you received the passport within the past 15 years
• you were over age 16 when it was issued
• you still have the same name, or can legally document your name change
